A Permission Form is needed for every youth activity that is not held on the New Hope Baptist Church campus.  A Current Emergency Medical form is also needed.  Emergency Medical Forms are kept on file for the calendar year.  As long as the information is current a new Emergency Medical form is not needed for each event.  Both forms are formatted so that you can type your information directly into the form fields.  It will not let you save the form to your computer, however, you may choose to print several copies at once to have them on hand for future youth events.
